Types Of Wisdom Tooth:
Wisdom teeth (third molars) can develop in different positions depending on available space and alignment:

Fully Erupted
A fully erupted wisdom tooth has completely emerged through the gum and may function like a normal molar if properly aligned and easy to clean; however, it can still cause problems such as decay, gum inflammation, or pressure on nearby teeth. If removal is necessary, the procedure is typically straightforward and less invasive than surgical extractions. After a clinical examination and X-ray, local anesthesia is administered, the tooth is gently loosened, and carefully removed with minimal trauma to surrounding tissues, allowing for a quicker recovery and reduced discomfort.

Partially Erupted
A partially erupted wisdom tooth is one that has only partially broken through the gum, leaving a flap of gum tissue covering part of the tooth. This condition creates a space where food particles and bacteria can easily become trapped, increasing the risk of infection, swelling, bad breath, and a painful condition known as pericoronitis. Patients may experience discomfort while chewing, jaw stiffness, or tenderness in the back of the mouth. Because partially erupted wisdom teeth are difficult to clean properly, they are more prone to decay and gum disease. In many cases, dentists recommend removal to prevent recurrent infections and protect the health of surrounding teeth and tissues.

Impacted – Soft Tissue
A soft tissue impacted wisdom tooth occurs when the tooth has partially emerged but remains trapped beneath the gum tissue. Although it may not be fully embedded in the jawbone, the surrounding gum prevents it from erupting properly. This condition can lead to pain, swelling, difficulty cleaning the area, and an increased risk of infection or pericoronitis. Food and bacteria can accumulate around the partially exposed tooth, causing inflammation and discomfort. Treatment usually involves a minor surgical procedure in which the gum tissue covering the tooth is gently opened and the tooth is carefully removed to prevent further complications and protect overall oral health.

Impacted – Bony Wisdom Tooth
A bony impacted wisdom tooth is completely or partially trapped within the jawbone and unable to erupt properly. This condition often occurs due to lack of space in the mouth or improper angulation of the tooth. Bony impactions can cause pressure, pain, swelling, jaw stiffness, and even damage to adjacent teeth. In some cases, cysts or infections may develop around the impacted tooth if left untreated. Removal typically requires a surgical procedure in which a small incision is made in the gum, a portion of the surrounding bone may be carefully removed, and the tooth is sectioned if necessary for safe extraction. Proper evaluation with dental imaging is essential to plan the procedure and minimize complications.

Angled (Mesial, Distal, Horizontal, or Vertical Impaction)
Angled impactions occur when a wisdom tooth grows in an abnormal direction rather than vertically like a normal molar. In a mesial impaction, the tooth tilts forward toward the neighboring molar, which is the most common type and may cause pressure or damage to the adjacent tooth. In a distal impaction, the tooth angles backward toward the jawbone. A horizontal impaction occurs when the tooth lies completely sideways, often pushing directly against the adjacent tooth and causing pain or crowding. A vertical impaction means the tooth is positioned upright but remains trapped beneath the gum or bone due to lack of space. These angled positions can lead to pain, swelling, decay in nearby teeth, cyst formation, or gum infection. Surgical removal is often recommended to prevent long-term complications and protect surrounding teeth.

FAQs
Most frequently asked questions
Wisdom teeth should be evaluated if you experience pain, swelling, infection, crowding, or if X-rays show impaction. Early removal can prevent future complications and damage to nearby teeth.
The procedure itself is performed under local anesthesia, so you should not feel pain during treatment. Mild discomfort or swelling after the procedure is normal and can be managed with prescribed medication and proper aftercare.
The cost of wisdom tooth removal depends on the complexity of the case. Simple extractions (fully erupted teeth) typically start from AED 200–500, while surgical removal of impacted wisdom teeth may range from AED 600 or more, depending on the level of difficulty and imaging required. A consultation is recommended to assess your condition and provide an accurate treatment plan and quotation.
